How to apply

You can request an individual contract or join one of our contracted independent practice associations or medical groups after meeting our credentialing criteria and standards.

Submit your credentialing application

Submit using the following options:

Note: Access to Provider Source ended on December 31, 2023. Starting January 1, 2024, submit your application using our hard-copy credentialing application while we transition to a new vendor. Find out more about the new credentialing vendor.

For dental and facility providers:

Facility/organization credentialing/recredentialing application

The credentialing process may take up to 60 days to complete.

Remember the following:

  • Current DEA certificate or coverage plan for the state where you practice
  • All current state licenses
  • Current professional liability insurance certificate (must not expire before credentialing is complete)
  • Any explanations to "Yes" answers on your credentialing application
  • Hospital Admission or Inpatient Coverage Plan
  • Make sure the attestation date on your provider application is within 150 days from when you submit your application.
  • Email us at credentialing.Updates@LifeWiseHealth.com to tell us you've completed your credentialing application and attach a copy of your IRS SS-4 confirmation letter showing your Employer Identification Number (EIN).

You'll receive a confirmation email from LifeWise that says we've received your request.

What happens next

Your acceptance is based on application information and network requirements. Please be aware that the credentialing process typically takes 45-60 days. If your submitted application is accepted, you'll receive a contract to review and sign through DocuSign. Once you return your signed contact, you'll receive a counter-signed contract through DocuSign with your participation effective date.

For all LifeWise plans, you can't see members until you're officially credentialed under contract. Also, your malpractice insurance must not expire before credentialing is complete.

Use your OneHealthPort account to sign in and access important LifeWise tools and resources.

Once you're signed in, you'll have access to member benefits and eligibility, claims status, forms, and more. Find everything you need in our Tools menu category.
