Starting January 1, 2020, LifeWise is offering a new network for our Individual Plans called LifeWise Primary. Customers can select EPO plans that give them access to all providers in the LifeWise Primary network. We expanded the LifeWise footprint to 33 counties this year.
Below are images of the new member ID cards for the Individual market which will help you identify these patients. For more information, see our ID card guide.
Individual member ID card:
- Network is LifeWise Primary
- Individual Plan is marked in the bottom right quarter of the card
- Member’s assigned primary care center is in the top right quarter of the card above network
- Express Scripts is the pharmacy benefit manager
- There’s a new customer service number and claims mailing address on the back of the card
- There’s no alpha prefix on the card
Grandfathered plan ID card:
- There are a handful of members with grandfathered Individual Plans
- Members with these plans have access to the LifeWise Health Plan of Washington Preferred network
Dental ID cards:
- Individual Plan members have access to the Dental Value network
