Our LifeWise network spans Washington state and Clark County and includes primary care providers, specialists, facilities, nurses, mental health therapists, and more. Find providers for referrals, etc. or check network status of any providers.

How to apply

You can request an individual contract or join one of our contracted independent practice associations or medical groups after meeting our credentialing criteria and standards.

Note: Access to ProviderSource ended on December 31, 2023. Starting January 1, 2024, submit your application using our hard-copy credentialing application while we transition to a new vendor. Find out more about the new credentialing vendor.

After completing your credentialing application through ProviderSource, check the application box indicating that you’re applying for participation with LifeWise.

Important: Make sure the attestation date on your provider application is within 150 days from when you submit your application.

For facility providers

To join our provider network as a facility, complete the facility/organization credentialing/recredentialing application and a W-9 form. Email us your completed documents. The credentialing process typically takes 30 days. Acceptance is based on your application information and network requirements. If your submitted application is accepted, you'll receive a contract to review and sign. Once you return your signed contract, you’ll receive a counter-signed contract and the effective date of your participation.

The credentialing process may take up to 60 days to complete.

To avoid delays, remember to:

  1. Include the following items:
    1. Current DEA certificate or coverage plan for the state where you practice
    2. All current state licenses
    3. Current professional liability insurance certificate (must not expire before credentialing is complete)
    4. Any explanations to "Yes" answers on your credentialing application
  2. Confirm that the attestation date on your provider application is within 150 days from when you submit your application.
  3. Email us at credentialing.Updates@LifeWiseHealth.com to tell us you've completed your credentialing application and attach a copy of your IRS SS-4 confirmation letter showing your Employer Identification Number (EIN). You'll receive a confirmation email that says we've received your request.

What happens next

Your acceptance is based on application information and network requirements. Please be aware that the credentialing process typically takes 45-60 days. If your submitted application is accepted, you'll receive a contract to review and sign through DocuSign. Once you return your signed contact, you'll receive a counter-signed contract through DocuSign with your participation effective date.

For all LifeWise plans, you can't see members until you're officially credentialed under contract. Also, your malpractice insurance must not expire before credentialing is complete.

Updating your provider information

If you need to update your information, complete and email the provider update form to LifeWise with any new information or changes to your current practice or payment structure. The federal No Surprises Act requires health plans to verify all provider directory data every 90 days. It also requires all providers and facilities to submit this information to in-network plans. Unverified providers may be removed from our directory.
